New Patient Forms »Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid as per the contract agreement. We proudly accept all PPO insurance plans, though we are an out-of-network provider. This means you can use your PPO benefits with us, but coverage and out-of-pocket costs may vary based on your plan. You will be responsible for any balance not covered by your insurance. As a courtesy, we will file claims with your insurance provider to simplify the process. If your plan includes orthodontic coverage, you may benefit from reduced personal costs.
